Paperwork for your First Appointment:

Please print out these two patient forms and bring them to your first appointment. If you have not filled them out ahead of time, please let Holly know and we will arrange for you to arrive 10-15 minutes before your scheduled time and you may fill them out in her office (as we are not using our waiting room at this time) . If you would like a copy of these mailed to you, please contact us.

Payment & Insurance Information:

* Insurance Plan Coverage:

Holly Hamilton/Healing Pathway will provide Physical Therapy services as an in-network PT provider for: Regence Blue Cross, Premera, LifeWise, Bridgespan and Washington L&I  If you are choosing to utilize your insurance benefits, please let Holly know PRIOR to your session so your benefits may be verified.  Several insurances require Prior authorization which takes time. Note that several plans require that you meet your deductible before PT benefits will apply. It’s ideal for you to be aware of your coverage. 

Holly will check your benefit plan prior to your first session and thus will need a your ID # and Date of birth 

For out of network coverage: If you would like to submit bills to your insurance carrier to try and receive out-of-network reimbursement you may let us know you desire a “superbill” at the start of your treatment. Note: Many plans require pre-authorization for PT services and in these cases, this option does not apply.


* Personal-Injury Coverage:

Motor Vehicle Accident coverage is accepted with an active PIP. We will do all billing for you and no copay is required. A physician referral is likely needed.  These services will be billed at Physical Therapy rates under medical coverage.


* Self Pay:

Holistic Therapy Services which combine integrative PT, massage and lifestyle medicine are offered for self-pay clients and will be paid for in-full at time of service.  At our first evaluation session, we will discuss your goals and treatment plan so that it matches your financial capacity.

To aim for accessibility while providing optimal care, these out-of -pocket rates are set at a lower cost, with longer treatment times, than is typical in the industry for 1-on-1 care with a Doctor of Physical Therapy/ Licensed Massage therapist with two decades experience.  Please inquire to learn more about self-pay rates.

  • New Patient Evaluation & Holistic Treatment (~90 min) 
  • Follow-up Holistic Healing Session (60 minutes)
  • Follow-up Holistic Healing Session (90 minutes)

For Holistic PT & Bodywork packages, please aim to utilize them within 10 weeks. This is a good option for those committed to a treatment plan.  These may be paid for in advance or at the first session of the package.

  • Holistic Therapy Package (Four 60-min follow-up sessions)
  • Holistic Therapy Package (Four 90-min follow-up sessions)

Services will be paid at time of service with cash, check, card, Venmo or PayPal.  (A $5 discount available for patrons paying with check or cash).

Physical Therapy Services and Physician Referrals:

You do not necessarily need a referral from a physician to start Physical Therapy: In the state of Washington, patients may see a Physical Therapist with “direct access”.  Even if you do not have a script, we find that a team approach to wellness is invaluable, and we are happy to collaborate with your other health care providers (with a simple form to complete to allow practitioners to communicate).

* If you have a referral, please bring this with you to your initial appointment

Potential referring physicians can include: Medical Doctor (MD), Naturopath (ND), Chiropractor (DC), Osteopathic Physician (DO), Physicians Assistant (PA), Licensed Nurse Practitioner (LNP), Midwife (CNM) and Acupuncturist (LAc).

Cancellation Policy

Please contact us ASAP to reschedule your appointment.
You are expected to attend all of your appointments as we are reserving this time specifically for you. If you need to cancel or reschedule an appointment, please do so 24-hours prior to your appointment time. A $50 fee will be charged for sessions missed without such prior notification. This fee will be due prior to your next treatment. We understand there are occasional emergency situations and we appreciate your consideration of our time.  If ‘life happened’ and you did not cancel with adequate notice and want to remit a payment, there is a link to do so on the scheduling page.  

Release of Medical Records

Medical records are confidential information. During or after medical treatment, you may need to obtain your records. In order for us to release medical records we need a completed and signed Release of Records form. Thank you for understanding and appreciating our efforts to keep your information private.
I aim to provide the best continuity of care for all my current patients and past patients. For patients continuing PT with another provider, I am happy to consult with a future PT to guide a smooth transition of care. Our office may send along medical records to other providers upon request and a signed medical release form.
